PATIENT MOVEMENT TRACKER

Scope

Company patient intake team needed a tool that would allow them to see all available beds and patient openings per program per location to include project discharge date and potential patient to fill the vacancy.

Problem

The company did not possess any software to track patient movement from arrival through patient step up/down across various service levels. The current tool was not automated nor where calculations formulated. This resulted in lots of human error on exactly how many open beds there were an on what day any specific bed would be open due to manual calculations.

Solution

The following tool was build that auto calculated a total bed number once a patient name was entered. Would calculate total discharges and step ups/downs per service level per location.

Results

Eliminated almost 100% of human error calculations and allowed clinical teams and the intake team to know exactly what patients where incoming, stepping up/down, or discharging. This decreased the days a bed sat empty by a full day. Which lead to improved census and financial results for the company.
